DISCLAIMER

By participating in the St Dominic’s College Football Program parents of participating students must agree to the following on behalf of their son:

  1. He is physically fit and able to participate in the sporting activities to take place during the Trials and I acknowledge that participation in football involves inherent risks of injury. I understand that St Dominic’s College will take reasonable steps to ensure the safety and wellbeing of participants. To the extent permitted by law, I release and indemnify the College, its staff and coaches from liability for injury, loss or damage arising from participation in the Trials.
  2. In the event that he needs medical attention during the Trials, I agree that he can be treated by a qualified emergency first aid staff member. I authorise the College to seek emergency medical, dental or hospital treatment for my son if required and agree to meet any associated costs.
  3. He will conduct himself in an appropriate manner at all times during the Trials and will not act improperly, violently or impolitely towards any other person or property. My son will comply with the College Code of Conduct and all directions from staff and coaches during the Trials.
  4. I hereby grant St Dominic’s College, without approval or compensation, to use my son/s image in any of the photographs or video recordings taken at the Trials and within the Program.
  5. I understand that I am required to pay a $120 Application for Enrolment Fee as part of the Enrolment process. I acknowledge that if my son is not accepted into the St Dominic’s College Football Program, the Application Fee is non-refundable.
  6. I understand that my son’s eligibility to participate in the St Dominic’s College Football Program fixtures, tournaments, tours and additional planned external activities is subject to payment of College Fees being up to date. Failure to pay College Fees, resulting in your account being in arrears, will result in your son’s ineligibility to participate in these events.

    In order to participate in the St Dominic’s College Football Program, on acceptance of your offer into the Program if successful at trials, your son is required to be an enrolled student at St Dominic’s College.
